THIS ROLE IS REMOTE HOWEVER TO BE CONSIDERED FOR THIS ROLE YOU MUST RESIDE IN OHIO in either Clinton, Clermont, or Hamilton County.

• Monday through Friday
• 8:00 AM - 5:00 PM EST
• Pay Rate is $35.75

Healthcare Consultant is responsible for telephonically and/or face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

Requirements

• Bachelor's degree in Social Work
• Active and in good standing LSW for Ohio
• 3 years of behavioral health or social work experience
• 1 year of case management experience
• Computer Saavy skills (Microsoft Office: Word, Excel, and Outlook)
• Must be willing and able to travel up to 50-75% of the time in Clinton, Hamilton, or Clermont County. Reliable transportation required. Mileage is reimbursed.
