One of our clients is seeking a Case Manager in Alexandria, VA.
CLIENT'S AVAILABLE HOURS * Part Time - 20 hours a week
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY * Provides support coordination services to individuals with IDD, aged 22 and older
* Provides linkage of services to individuals and their family.
* Coordinate and monitor the delviery of services to live, work, and recreate in the least restrictive environment.
CLIENT'S REQUIRED SKILLS & EXPERIENCE * BA degree in Human Service field
* Minimum one year experience in mental health or substance abuse services
* Some experience at the level of Case Worker
